A spinal disc herniation is a medical condition affecting the spine in which a tear in the outer, fibrous ring of an intervertebral disc allows the soft, central portion to bulge out beyond the damaged outer rings. Disc herniation can occur with a traumatic injury causing a tear of the posterior longitudinal ligament in the spinal canal. This tear in the disc ring may result in the release of inflammatory chemical mediators which may directly cause severe pain, even in the absence of nerve root compression.
Although most minor herniation’s heal within several weeks and anti-inflammatory treatments for pain associated with disc herniation, protrusion, bulge, or disc tear are generally effective, severe herniation’s may not heal of their own accord and may require surgical intervention
The posterior longitudinal ligament is situated within the vertebral canal, and extends along the posterior surfaces of the bodies of the vertebra, from the body of the axis, where it is continuous with the membrana tentoria, to the sacrum.
It is broader above than below, and thicker in the thoracic than in the cervical and lumbar regions. The ligament is more narrow at the vertebral bodies and wider at the intervertebral disc space which is more pronounced than the anterior longitudinal ligament. This is significant in understanding certain pathological conditions of the spine such as the typical location for a spinal disc herniation.
This ligament is composed of smooth, shining, longitudinal fibers, denser and more compact than those of the anterior ligament, and consists of superficial layers occupying the interval between three or four vertebrae, and deeper layers which extend between adjacent vertebra.
